A GUIDE FOR PEOPLE EXPERIENCING HOMELESSNESS

Cook County Birth Certificates

Birth certificates are free for people born in Cook County who are homeless, survivors of domestic violence, or have been recently released from Illinois Department of Corrections or the Cook County Jail. This form asks you to prove your homeless status, and can be filled out by a homeless service provider or an attorney. If you were born in Cook County and would like to request a free birth certificate us this form.

Other Illinois Counties Birth Certificates

Birth certificates are also free for people born in other counties in Illinois. To obtain a free birth certificate from another County, you must use this form, which has to be signed by a homeless service provider or an attorney.

State IDs

Illinois State IDs are also free for people who are homeless in Illinois. You will be required to prove your identity, but this form serves as proof of residency and waives the fee for the ID. It must be filled out by a homeless service provider or an attorney.
